Job Description Summary

We are seeking a highly skilled and compassionate Clinical Manager to join our dedicated hospice team. As the Clinical Manager, you will play a crucial role in ensuring the delivery of high-quality patient care within the hospice setting. Collaborating closely with the Administrator, you will oversee and coordinate nursing activities, maintaining compliance with regulatory standards and fostering a supportive environment for both patients and staff.

The Clinical Manager is responsible for ensuring that patient care is coordinated and managed appropriately, and for the supervision of clinical personnel.

Essential Job Functions/Responsibilities
  • Provide leadership and supervision to nursing and clinical personnel, ensuring the delivery of compassionate and patient-centered care.
  • Conduct regular assessments of patient needs and collaborate with the interdisciplinary team to develop comprehensive care plans.
  • Monitor and evaluate the performance of nursing staff, providing guidance, feedback, and support as needed.
  • Foster a positive and collaborative work environment that promotes professional growth and development for clinical personnel.
  • Ensure compliance with state and federal regulations, as well as accreditation standards, by regularly reviewing and updating policies and procedures.
  • Participate in quality improvement initiatives, conducting audits and assessments to enhance the overall quality of patient care.
  • Collaborate with other healthcare professionals to optimize the continuum of care for hospice patients.
  • Assist in the recruitment, orientation, and training of nursing staff.
  • Actively engage in ongoing education and professional development to stay abreast of industry trends and best practices.
  • Receives case referrals. Reviews available patient information related to case, including home visits, to determine hospice/home care needs. Assigns appropriate hospice personnel to case as needed. Conferences with MEDICAL DIRECTOR regarding any questions about an individual s eligibility for services.
  • Reviews and evaluates each case through a variety of means such as home visits, conferences, record review and the services provided by clinicians; discusses and verifies impressions, instructs and guides clinicians to promote more effective performance and delivery of quality home care services; and is available at all times during operating hours to assist clinicians as appropriate.
  • Reviews patient s medical diagnosis, prognosis, medications, procedures and clinical course.
  • Assists clinicians in establishing immediate and long-term therapeutic goals, in setting priorities, and in developing plan of care.
  • Attends case conference meetings with hospice personnel to facilitate coordination of care and discussion of interdisciplinary group involvement.
  • Conducts quarterly record reviews and communicates findings and recommendations to Administrator and hospice personnel.
  • Assists in the screening and interviewing process of new employees and makes recommendations for employment of individuals. Assists in the orientation of new employees.
