Xclusive Care Services, LLC is a trusted leader servicing medically fragile children in Georgia. We are a caring family partner providing personal assistance and medical care.

• Provide direct nursing care, including administering medications, managing ventilators, feeding tubes, tracheostomies, and other medical devices.
• Conduct supervisory visits every two weeks (plus monthly, or every 60 days) to our patients at their homes.
• Monitor and assess the child’s condition regularly, recognizing and responding to any changes in status.
• Perform and document clinical assessments, including vital signs, pain management, and overall well-being.
• Implement and follow the child's care plan as prescribed by the physician.
• Collaborate with the healthcare team and other nurses to ensure comprehensive care.
• Communicate any changes in the child’s condition to the physician and healthcare team promptly.
• Maintain accurate and thorough documentation of the child’s care, including progress notes, medication records, and other required documentation.
• Provide supervisory management to parent caregiver.
• Maintain accurate, detailed reports and records.
• Administer medications to patients and monitor patients for reactions or side effects.
• Record patients' medical information and vital signs.
• Monitor, record, and report symptoms or changes in patients' conditions.
• Modify patient treatment plans as indicated by patients' responses and conditions.
• Monitor all aspects of patient care.
• Prepare patients for and assist with examinations or treatments.

• Education:
  
  Bachelor's degree in nursing (Preferred) or LPN.
• License: RN License in the state of Georgia or LPN; CPR Certification (Required).
• Experience:
  
  1–2 years of related experience, or an equivalent combination of education and experience.
• Excellent communication skills and professional demeanor.
• Must be dependable and have reliable transportation with a current driver’s license.

• Job Types: Part-time, Contract.
• Pay:
  Up to $4,000 a Month.
