Job description:

Xclusive Care Services, LLC is a caring family partner; we provide home-based care for medically fragile children in Georgia.

• Provide direct nursing care, including administering medications, managing ventilators, feeding tubes, tracheostomies, and other medical devices.
• Conduct supervisory visits: bi-weekly, monthly, or every 60 days, to patients at their homes.
• Monitor and assess child’s condition regularly; recognize and respond to changes.
• Perform and document clinical assessments: vital signs, pain management, overall well-being.
• Implement and follow the child’s care plan as prescribed by the physician.
• Coordinate care: collaborate with healthcare team and other nurses for comprehensive care.
• Communicate changes in patient’s condition to physician and healthcare team promptly.
• Maintain accurate documentation: progress notes, medication records, required docs.
• Provide supervisory management to parent caregiver.
• Maintain accurate, detailed reports and records.
• Administer medications and monitor patients for reactions or side effects.
• Record patients’ medical information and vital signs.
• Monitor, record, and report symptoms or changes in patients’ conditions.
• Modify treatment plans based on patient response.
• Monitor all aspects of patient care.
• Prepare patients for and assist with examinations or treatments.

• Bachelor’s degree in nursing (preferred) or LPN.
• RN license in the state of Georgia or LPN.
• CPR certification (required).
• 1-2 years of related experience, or equivalent combination of education and experience.
• Excellent communication skills and professional demeanor.
• Dependable with reliable transportation and current driver’s license.

Job Types: Part-time, Contract

Pay:
Up to $4,000 a month
