The Children’s Medical Services program is seeking a highly qualified candidate to fill the full‑time, salaried position of PH RN Case Manager, based in Statesboro, GA.

Job ResponsibilitiesUnder general supervision, following established policies, procedures, and protocols to provide comprehensive support to families of children with special healthcare needs. The role involves developing and managing individualized care plans, coordinating ongoing care, and facilitating transition planning from age 12 onward. It includes actively engaging and retaining families in the program while promoting CMS services throughout the district, building and maintaining relationships with community partners, participating in outreach activities, serving as the primary case manager for families in Candler, Tattnall, Jeff Davis, Appling, Wayne, Bulloch and Toombs counties, managing enrollments, conducting six‑month and annual reviews, ensuring continuity of care, reviewing medical records, approving billing and documentation for other nurses, and assisting with phone inquiries as needed.
Success requires strong organizational skills, effective communication, and a commitment to improving outcomes for families with special healthcare needs.
- Associate degree in nursing from an accredited college or university.
- Completion of a PHN course within one (1) year of hire.
- Current Georgia Registered Professional Nurse license.
- CPR certification.
- Experience working with infants or young children.
- Bachelor’s degree in nursing (BSN).
- One year minimum experience as a registered professional nurse.
- Bilingual proficiency in Spanish.

Current state employees are subject to State Personnel Board rules regarding salary. The district accepts educational credentials recognized by CHEA or the U.S. Department of Education. The selected candidate may be subject to pre‑employment drug screening and a criminal background check. Employees may be required, as a term and condition of employment, to assist in meeting emergency responsibilities of the department.
