Job Title
RN Case Manager extraordinaire (RN Case Manager)
Responsibilities- Be a steady source of comfort and support to patients and families
- Help families understand disease progression, self‑care, and nutrition
- Collaborate with the interdisciplinary hospice team to create and follow patient care plans
- Manage symptoms to promote patient comfort and advocate for patients
- Maintain timely and accurate clinical documentation
Compassionate, dedicated RN Case Manager
- Valid GA RN license
- Driver’s license
- At least one year of hospice or home health experience
