Soleo Health is seeking a Licensed Practical Nurse (LPN) to support our Specialty Infusion patients in Norcross, GA

- Provide the majority of direct care for assigned patients, including initiation and maintenance of all communications with relevant care team members.
- Evaluate and educate patients/ caregivers to safely administer home infusion therapy.
- Assess the home environment to identify required interventions for safety and appropriateness.
- Develop patients’ Care Maps in collaboration with clinical team members and evaluate response to therapy during each visit.
- Discharge patients appropriately and complete discharge summaries within 7 days.
- Maintain caseload scheduling and communications, updating schedules weekly and monthly.
- Document patient visits to physicians or labs on the calendar.
- Coordinate care via phone or voicemail based on urgency.
- Document all communications promptly.
- Assist with vital sign monitoring, PICC dressing changes, PIV insertions, and other administrative tasks per state nursing laws.
- Full‑time, 40 hours per week, Monday – Friday, 8:30 a.m. – 5:00 p.m.
- Licensed LPN in the state of Georgia.
- IV Certified or willing to become IV Certified.
- Graduate of an accredited LPN or LVN nursing program.
- Minimum 1 year of nursing experience, with background in infusion therapy preferred.
- Current LPN license in good standing.
- Excellent customer service skills.
- IV Certification required.
- Proficiency with Microsoft Excel, Word, and Outlook.
