Soleo Health is seeking a Licensed Practical Nurse (LPN) to support our Specialty Infusion patients in Norcross, GA

• Provide the majority of direct care for assigned patients, including initiation and maintenance of all communications with relevant care team members.
• Evaluate and educate patients/ caregivers to safely administer home infusion therapy.
• Assess the home environment to identify required interventions for safety and appropriateness.
• Develop patients’ Care Maps in collaboration with clinical team members and evaluate response to therapy during each visit.
• Discharge patients appropriately and complete discharge summaries within 7 days.
• Maintain caseload scheduling and communications, updating schedules weekly and monthly.
• Document patient visits to physicians or labs on the calendar.
• Coordinate care via phone or voicemail based on urgency.
• Document all communications promptly.
• Assist with vital sign monitoring, PICC dressing changes, PIV insertions, and other administrative tasks per state nursing laws.

• Full‑time, 40 hours per week, Monday – Friday, 8:30 a.m. – 5:00 p.m.
• Licensed LPN in the state of Georgia.
• IV Certified or willing to become IV Certified.

• Graduate of an accredited LPN or LVN nursing program.
• Minimum 1 year of nursing experience, with background in infusion therapy preferred.
• Current LPN license in good standing.
• Excellent customer service skills.
• IV Certification required.
• Proficiency with Microsoft Excel, Word, and Outlook.
