Pruitthealth Corporation is seeking an LPN Assessment Nurse to perform initial and reassessment visits for elderly and disabled waiver members in Carroll & Douglas counties. Responsibilities include home assessments, medication review, and determining service needs.
The role requires prior LPN licensure, at least two years of practical nursing experience, and ability to coordinate with Medical Director and Home First teams. Community-based care is provided in members' homes.
