Presidential Home Care LLC is looking for a compassionate and dedicated Licensed Practical Nurse (LPN) to join our growing team in Norcross, GA. Responsibilities- Provide direct patient care including wound care, medication administration, and vital sign monitoring
- Develop and follow individualized patient care plans in coordination with supervising RNs and physicians
- Observe and document patient health status, progress, and any changes in condition
- Assist patients with activities of daily living (ADLs) and personal hygiene as needed
- Educate patients and family members on care procedures, medications, and health management
- Communicate effectively with the healthcare team to ensure continuity of care
- Maintain accurate and timely clinical documentation in compliance with state and agency regulations
- Active Licensed Practical Nurse (LPN) license in the state of Georgia
- Prior home health, long-term care, or clinical nursing experience preferred
- Strong knowledge of nursing procedures, medication management, and patient assessment
- Excellent communication, organizational, and time-management skills
- Ability to work independently and exercise sound clinical judgment
- Valid driver's license and reliable transportation required
- CPR/BLS certification preferred
