Summary:
Under the direction of the registered nurse (RN), provides nursing care in accordance with departmental and hospital policies and procedures as well as with their State Practice Act. The licensed practical nurse (LPN) assists the RN with assessing, planning, implementing, and evaluating patient care. The LPN is responsible for documentation of their care including interventions, outcomes, and patient/family education. They collaborate with physicians and other members of the interdisciplinary health care team to provide continuity of care.
The LPN is responsible for the direction and assignment of nursing interventions that may be performed by others as well as care and interventions delegated to them by the RN. Must demonstrate the knowledge and skills necessary to provide age-appropriate patient care and is accountable to the policies and procedures of the organization.

1. Demonstrates ability to appropriately modify approach and procedures to meet needs of age/diversity of population served for the following age groups:
☐ Neonate (birth - 28 days)
☐ Infant (29 days - less than 1 year)
☐ Pediatric/Child (1 year - 12 years)
☒ Adolescent (12 years - 18 years)
☒ Adult (18 years - 65 years)
☒ Geriatric (over 65 years)
- Incorporates cultural considerations in the provisions of care.
- Knowledge of growth and developmental stages.
- Considers life changes/effects on health beliefs and behaviors.
- Provides necessary safety measures.
- Provides information and involves family/caregiver in decision making.
- Contributes to patient assessment, prioritizing the data collection based on the patient's immediate condition or needs within the timeframe specified by policy.
- Updates and documents patient's plan of care, once it is established by the RN.
- Upon admission and as appropriate, assists the RN with patient/family/others discharge planning and teaching needs.
- Delegates and assigns appropriate patient care to aides.
- Recognizes changes in patient status based on interpretation of assessments. Reports findings appropriately and timely.
- Performs therapeutic nursing interventions in line with the plan of care.
- Documents all nursing cares and education accurately and timely in the electronic health record.
- Documents response to nursing cares and education accurately and timely in the electronic health record.
- Demonstrates knowledge of hospital policies and procedures for administering, verifying, and recording medications.
- Demonstrates knowledge of commonly used meds: action, dose, and side effects.
- Functions successfully in an emergency situation. Maintains professional composure and confidence during stressful situations.
- Identifies patient care problems and communicates them to RN
- Provides care in a non-judgmental, non-discriminatory manner that is sensitive to patient and family diversity, preserving autonomy, dignity, and rights.
- Actively participates and involves the patient and family in handoffs and bedside shift reports and relates pertinent patient information.
