About the Norfolk Community Services Board (NCSB)

Norfolk Community Services Board (NCSB) provides treatment, support, and education to the community with a focus on mental illness, substance use addiction, and intellectual/developmental disabilities. NCSB offers services to persons in acute crisis as well as integrated, person‑centered, and recovery‑oriented programming for those needing assistance and support for community stabilization. Today, the NCSB provides treatment and rehabilitation services to over 6,000 Norfolk citizens each year.

Job Summary

The NCSB is seeking a Case Manager III to provide street and site outreach to homeless individuals and families. The role involves assisting consumers in accessing housing, medical, social, educational, vocational, and other supports; completing documentation in accordance with agency guidelines; and promoting a positive work and treatment environment.

Department Salary Range

$54,190 – $62,185

Essential Functions
  • Conduct street outreach to homeless individuals in Norfolk and develop relationships with people experiencing homelessness.
  • Perform face‑to‑face assessments to determine functional limitations, eligibility for services, and need areas through street contacts, site visits, office visits, or other community settings.
  • Determine eligibility for enrollment and enroll consumers as appropriate; develop a Plan of Action based on assessment information; review and update the plan as needed.
  • Link consumers to services, resources, and support specified in the Plan, including housing, entitlements, medical care, mental health services, substance use services, food, skill training, etc.
  • Assist individuals directly, including transportation, to obtain needed resources and advocate for their procurement while empowering independent living.
  • Coordinate services and planning with other NCSB providers and external agencies, ensuring continuity of care and obtaining clinical documentation.
  • Contact family or significant others to promote implementation of the Individual Service Plan and community stability.
  • Monitor progress, service delivery, physical and mental stability, and consumer satisfaction; provide monthly services and increased frequency when clinically indicated.
  • Provide practical guidance and education, encouraging compliance with treatment, discussing relapse prevention, stress management, and resolving critical situations.
  • Maintain client records in compliance with agency guidelines and state regulations; complete accurate daily service records.
  • Comply with regulations for HIPAA, mandated reporting of abuse and neglect, consumer confidentiality, and other consumer rights.
  • Attend agency staff meetings, case review meetings, supervisory meetings, and training as required.
  • Perform other related duties as assigned.
Education & Experience Requirements
  • Bachelor's degree in a human services field.
  • Two years of experience in a human services field related to the assignment; some assignments may require specific experience working with various client populations.
Preferred Education & Experience
  • Bachelor's degree or equivalent combination of training and experience in a human services field.
  • Experience in case management and working with individuals experiencing homelessness.
  • At least two years of direct experience providing services to individuals experiencing homelessness.
