Description and Responsibilities
Community Connections is a Day Habilitation program that serves adults between the ages of 23-66 with a primary diagnosis of intellectual disabilities, though some individuals may also have mental health diagnoses and/or physical challenges. Most participants require supportive structures and staff assistance to maximize their involvement in activities within the program as well as within the local community. Our staff create individualized service plans, daily curriculum/activities, and focus heavily on community integration.
The Day Hab Director/Service Manager is responsible for oversight of day‑to‑day activities and delivery of services at the Community Connections Day Hab Program. They provide leadership, supervision, and support to the team of Developmental Specialists and Teaching Assistants, ensuring implementation of service plans, compliance with Mass Health regulations and funder requirements. The Day Hab Director/Service Manager collaborates with clinical staff, nursing, and external consultants to ensure individuals receive optimal support, are educated about their rights, and are provided advocacy on their behalf when needed.
OtherKey Responsibilities
- Develop, coordinate, and oversee the implementation of Day Hab Service Plans (DHSP) and supported routines including training.
- Hire, supervise, mentor, and evaluate staff.
- Facilitate and participate in supervision, team, and other meetings. Ensure staff meet all training requirements.
- Maximize the development of community‑based programming and volunteer activities. Obtain any resources needed to support skill and goal achievement in the program.
- Assist in daily programming by providing group coverage, crisis intervention, and other related tasks as needed. Provide case management as assigned.
- Develop, coordinate and oversee systems for program monitoring, data collection, reporting, and other methods to be followed by staff.
- Bachelor’s Degree in a health‑related field and a minimum of three years relevant experience in healthcare, required. Six years of relevant health care experience, with three of those years serving in a supervisory role, may be substituted in lieu of a bachelor’s degree.
- Minimum of two years previous supervisory experience, required.
- Valid Driver’s License and acceptable driving record, required.
- Reliability, willingness to learn, and openness to new opportunities.
