Inpatient Case Manager-RN

Overview

Sentara Norfolk General Hospital is hiring an Inpatient Case Manager RN to work full-time days. A generous sign-on bonus is available for qualified candidates.

• Provide and facilitate comprehensive care management, services and quality outcomes for patients across the healthcare continuum, with a primary focus on patient transition to the appropriate level of care while supporting clinical appropriateness and length of stay (LOS).
• Directly support hospital inpatients identified as needing comprehensive case management.
• Promote effective utilization and monitoring of health services; collaborate and communicate with the healthcare team and patient/caregiver to manage care and transitions.
• Develop and/or implement a comprehensive, multidisciplinary care plan based on assessment and evaluation to address physical, mental, and psychosocial needs of the inpatient/patient caregiver.
• Function in one of the following practice settings:
  Hospitals.

• 12 months of nursing experience (Registered Nurse).
• Basic Life Support (BLS) within 90 days of hire.
• BSN preferred.
• Case Management certification required within one year of eligibility (ACM, CCM, CCCTM, CMAC or CMGT-BC).
• 1 year acute care case management experience preferred.
