Position Type:
Full-Time, 1st Shift – Monday to Friday 8:00 AM–4:30 PM with rotating on‑call coverage and weekend community visits.

• Licensed Practical Nurse (LPN) with an active Virginia Board of Nursing license.
• Two years of experience providing mental health services to adults with serious mental illness.
• At least one year of experience in medication management.
• Strong knowledge of recovery principles and recovery‑driven care.
• Valid Virginia driver’s license.

We’re currently seeking a Licensed Practical Nurse (LPN) to join our Assertive Community Treatment (ACT/FACT) team.

Starting rate of $30.30 per hour + mileage reimbursement.

• Provide recovery-based, person‑centered clinical services through individual and group therapy, supporting mental health, daily living skills, and social development.
• Develop, implement, and regularly update treatment plans in collaboration with individuals, families, and the interdisciplinary team.
• Conduct comprehensive assessments and provide ongoing evaluation of symptoms, treatment responses, and overall progress.
• Support overall health needs by providing medication education, monitoring, administration, and addressing physical health concerns.
• Act as a liaison and advocate by coordinating with families and community agencies, offering education, crisis support, and connections to resources.
