Overview
Integrated Nurse Case Manager/Registered Nurse (RN) for Chronic/Complex Condition roles in Norfolk, VA. Full‑time, work‑from‑home with travel for home visits (approx 2–3 visits per week).
Hours:
8 am–5 pm EST, Monday – Friday. Must reside in the Norfolk/Virginia Beach area.
- Manage case management services within scope of RN licensure; develop, monitor, evaluate, and revise member care plans to optimize health across the continuum.
- Conduct telephonic or face‑to‑face clinical assessments to identify, evaluate, coordinate, and manage member needs across physical, behavioral, social, and long‑term services.
- Identify members at high risk for complications and coordinate care with members and care teams.
- Manage chronic illnesses, comorbidities, or disabilities to ensure cost‑effective and efficient use of benefits; conduct gap‑in‑care reviews for quality programs.
- Assist implementation of care plans by facilitating authorizations/referrals within benefit structures or extra‑contractual arrangements as permissible.
- Interface with Medical Directors, Physician Advisors, and Inter‑Disciplinary Teams on treatment plans.
- Present cases at multidisciplinary case conferences; ensure compliance with regulatory, accrediting, and company policies.
- Assist in problem‑solving provider, claims, or service issues as needed.

- Members diagnosed with chronic/complex conditions (Asthma, diabetes, heart failure, cancer, heart disease, COPD)
- Associate or Bachelor’s Degree in Nursing (required)
- Registered Nurse (RN) license – Compact or Virginia (required)
- Minimum 3 years nursing experience (required); case‑management experience highly preferred
- Experience with managed‑care or health‑plan settings preferred
- Experience working with chronic/complex condition members preferred (asthma, diabetes, heart failure, cancer, heart disease, COPD)
- Experience in home health, inpatient case management, hospice, transition of care, or discharge planning preferred
- Strong knowledge of physical, psychological, socio‑cultural, and cognitive patient needs
- Excellent oral and written communication; strong problem‑solving and analytical skills
