Overview
The RN - Transplant Coordinator coordinates solid organ (kidney) transplantation of all pediatric/young adult organ recipients. The Transplant Coordinator provides follow-up and care across the continuum from pre-transplant, during transplant, and post-transplant. The position collaborates with members of the health care team to enhance patient care by developing, supporting and educating nursing staff. Reports to departmental leadership.
Essential Duties and Responsibilities- Provides nursing care for pediatric and young adult (birth to 21 years) patients and families before and after kidney transplantation based on current best practices and research in transplant care, along the continuum of outpatient to inpatient, 24 hours a day, 7 days a week.
- Coordinates evaluation process for pre-transplant patients to ensure effective use of hospital and clinical resources.
- Interprets eligibility criteria for patients, parents/guardians, families, physicians, and outside agencies.
- Assesses post-transplant organ function and general health status and collaboratively determines need for additional assessment or intervention.
- Participates in the development and implementation of protocols and standards of care for the transplant patient in collaboration with the multidisciplinary team.
- Assists nurses and other staff to assess, plan, implement and evaluate the care of the transplant patient.
- Monitors the process and outcomes of routine care to determine quality improvement needs.
- Provides on-call coverage, including coordination of activities prior to transplant, in collaboration with the physicians on-call including the coordination of the transplantation process when a donor is available (24 hours/7 days a week).
- Completes necessary documentation according to the Organ Procurement and Transplantation Network (OPTN)/ Centers for Medicare & Medicaid Services (CMS) regulations including listing, status changes and post-transplant follow-up information.
- Collaborates with team members in coordinating discharge plans for patients’ post-transplant management including, but not limited to, referral to home health and infusion therapy.
- Educates patients/guardians, multidisciplinary staff, and nursing staff along the continuum to assist understanding of processes and procedures surrounding pre- and post-transplant care issues.
- Develops education materials or identifies external resources that are appropriate for parent/guardian and patient education in all phases of transplantation care.
- Participates or leads team conferences, rounds, meetings, in relation to care of the transplant patient.
- Participates in unit, departmental, and hospital-wide committees, task forces, or meetings as indicated by role appropriateness.
- Performs all other duties as assigned.
- Required Licenses and/or Certifications
- Current Virginia state license as a Registered Nurse (RN) or Registered Nurse holding a valid Compact State license required.
- Certified Clinical Transplant Coordinator (CCTC) required within three (3) years of hire.
- Preferred Licenses and/or Certifications
- Certified Pediatric Nurse (CPN) preferred.

Education and Experience- Bachelor of Science in Nursing (BSN) or relevant area from an accredited college or university or equivalent combination of experience and education.
- Technical and professional nursing knowledge, skills, and abilities acquired through a minimum of two (2) years of progressive acute care experience in solid organ transplant, critical care, or end-stage kidney disease.

Education and Experience- Two (2) years of solid organ transplant related nursing experience
- Two (2) years of pediatric nursing acute care experience
- Master of Science in Nursing (MSN) or relevant area preferred
