LPN - Inpatient Case Management

Overview

Sentara Norfolk General Hospital is hiring an LPN Intake Coordinator – Inpatient Case Management to work part‑time weekend day shift (20 hours per week) in Norfolk, VA.

• Triage incoming patients and verify medical information.
• Coordinate case management activities and liaise with physicians and other team members.
• Provide patient care support and ensure overall health needs are addressed.

• Licensed Practical Nurse, active in the state of Virginia.
• Minimum 1 year of clinical experience.
• Minimum 2 years of customer service experience.
• BLS certification required within 90 days of hire.
