Position: Family Nurse Practitioner / Physician Assistant

Job Overview

In this role, you will evaluate and treat patients across the lifespan, manage acute and chronic conditions, order and interpret diagnostics, and provide patient education in a collaborative, patient-centered environment. Responsibilities
  • Provide comprehensive primary and urgent care to patients across the lifespan in a clinic setting.
  • Conduct histories, physical exams, and develop evidence-based treatment plans.
  • Order, interpret, and act on diagnostic tests and imaging.
  • Prescribe medications and manage chronic conditions per guidelines and protocols.
  • Deliver patient and family education focused on prevention and self-management.
  • Collaborate with physicians, nurses, and support staff to optimize patient outcomes.
  • Document all encounters accurately within the EHR in a timely manner.
  • Participate in quality improvement, clinical protocols, and compliance initiatives.
  • Support clinic workflow, including triage and same-day patient needs.
  • Contribute to a culture of compassion, safety, and community-focused care.
  • Primary care assessment and treatment.
  • Urgent care and episodic illness management.
  • Chronic disease management.
  • Pediatric and adult patient care.
  • Ordering and interpreting labs and imaging.
  • Electronic health records (EHR) documentation.
  • Medication management and prescribing.

Qualifications
  • Active, unrestricted state license as Family Nurse Practitioner (FNP) or Physician Assistant (PA).
  • National certification (AANP/ANCC for FNP or NCCPA for PA).
  • Current DEA registration (or eligibility).
  • Current BLS certification; ACLS preferred.
  • Immunization and TB clearance per employer policy.
  • Experience with EHR systems.
  • Eligibility to work in the United States.
