**** Sentara Heart Hospital located on the campus of Sentara Norfolk General Hospital in Norfolk, Virginia is hiring a Transplant Coordinator Registered Nurse for the Advanced Heart Program!
** The Transplant Coordinator plays a critical role in facilitating the complex logistics of organ transplantation while prioritizing patient safety and well-being while ensuring a smooth and efficient journey for both the donor and recipient.
The Transplant Coordinator coordinates the clinical management of the transplant/VAD patient including the transplant/VAD evaluation, the transplant/VAD process and follow-up care. Provides clinical leadership to care team members, allowing staff to plan, direct, manage outcomes and evaluate care.
** Advanced Heart Failure Unit:
*** 24 bed Specialty Unit that provides high acuity cardiac care to patients with severe heart failure, pre/post heart transplant, the Total Artificial Heart, and pulmonary hypertension.
* Work with high tech equipment such as mechanical circulatory support including ventricular assist devices (LVAD, BiVAD) and telemetry monitors.
* Environment relies on 24 hours consistent communication and intricate interdisciplinary teamwork skills.
* Nurses handle 3-4 patients on this unit based on acuity level.
** Education
* ** Bachelor of Science Nursing- BSN (required)
** Certification/Licensure
*** Virginia RN License Virginia or Compact/Multi-State Eligible.
* All Direct Care RN's required to have BLS within 90 days of hire.
* Certified Clinical Transplant Coordinator (CCTC) required within three (3) years of hire.
** Experience
* ** Two (2) years of related nursing or transplant experience required.,.
