Overview
Full Time/Day Shift Transplant Coordinator role for the Kidney/Pancreas Transplant team at Sentara Norfolk General Hospital in Norfolk, VA. The position focuses on coordinating patient care during and after transplant, providing education, and managing the transplant process from admission to discharge and ongoing support.
Location & ShiftNorfolk, VA. First (Days) shift.
Responsibilities- Coordinate care with post-transplant patients.
- Facilitate clinic flow, transplant evaluation, and patient care management.
- Provide weekly education sessions to patients and supporters; arrange appointments with surgeons, social workers, financial coordinators, pharmacy, and dietary services.
- Admit patients, support them and their caregivers during the waiting period, and discharge transplant patients with home care instructions.
- Manage a caseload of patients, remain the patient’s point of contact throughout the transplant process, recovery, and home follow‑up.
- Conduct patient and family support during the post‑transplant period and coordinate community resources as needed.
- Registered Nurse, BSN with 2+ years acute care experience and strong medical‑renal knowledge.
- BLS certification required.
- Certified Clinical Transplant Coordinator (CCTC) required within three (3) years of hire.
