In this role, you will develop and ensure the wound care plan specific for each patient in collaboration with an interdisciplinary team. The Wound/Treatment Nurse plays a critical role in providing superior customer service and nursing care to all Residents. The position will obtain treatment orders from attending physicians and assist with modifying the treatment regimen in accordance with established policies and procedures.

• Complete a Head to Toe body audit on all new admissions and re-admissions.
• Ensure skin sheets, treatment orders, and preventative measures (T&P, specialty mattress, heels up or boots) are in place.
• Review residents with new skin alterations and implement skin sheets, treatment orders, and care plans.

• Update skin sheets every 7 days.
• Unit Managers update care plans weekly for pressure and non-pressure areas by Friday.
• Submit wound report to VP of Clinical Services by Thursday (percentage of Center Acquired Pressure Ulcers).
• Complete treatments for vascular, surgical, or pressure wounds of stage 2 or greater Monday–Friday.
• Ensure skin tears stage 1 are treated by Charge Nurse Monday–Friday and weekly skin sheets completed.

• Prevent wounds: provide daily education to staff about T&P, toileting, incontinent care.
• Change treatment within 2 weeks when wounds worsen, in consultation with the wound physician and PCP.
• Ensure preventive items for residents at risk of Center Acquired Pressure Ulcers (orders for T&P, preventive barrier cream, pressure-relieving devices).
• Assist nursing team with skin care treatment planning and consultations.
• Delegate care and supervise C.N.A.s and subordinate staff.
• Administer medications within scope of practice and maintain narcotic logs.
• Chart and document resident status and progress.
• Maintain infection control and safety precautions.
• Maintain resident comfort, privacy, and dignity.
• Answer call lights promptly and courteously.
• Assist with admissions and re-admissions when directed.
• Communicate effectively with residents, families, peers, and supervisors.
• Attend departmental meetings and in‑services as directed.

• Graduate from an accredited nursing program.
• Current State License as a Registered Nurse or Licensed Practical Nurse.
• CPR Certified.
• LTC/SNF experience preferred.
