• Serve as a primary care provider for geriatric patients located in long-term care facilities.
• Provide a low volume of patient care per day, allowing ample time to focus on all needs and provide proper attention and support for patients.
• Plan, direct, and evaluate patient medical care.
• Examine and treat chronic and urgent illnesses, minor injuries, etc.
• Order and interpret laboratory and diagnostic testing.
• Collaborate with caregivers and other healthcare professionals.
• Provide interdisciplinary care.
• Maintain a comprehensive plan of care.
• Provide referrals as necessary.
• Support the practice’s mobile‑based EHR platform with dedicated team assistance (orders, referrals, phone messages, etc.).
• Share on‑call responsibilities (phone triage only, light volume) limited to no more than 3 nights per month, with schedule flexibility.

• Virginia NP license
• DEA registration
• 1+ year advanced practice experience
• Experience in primary care, geriatrics, acute care, etc. preferred
