Silver Care is seeking a compassionate, dependable, and experienced Part‑Time Home Care Nurse to join our team!
This position is ideal for a nurse looking for a flexible part-time opportunity providing in‑home assessments, supervisory visits, care planning, and caregiver support throughout the Hampton Roads area.

• Windsor and surrounding areas
- Conduct initial client assessments and ongoing supervisory visits
- Assess client needs and develop or update individualized care plans
- Complete required client documentation and paperwork accurately
- Communicate with Silver Care staff and community partners regarding client needs and service authorizations
- Monitor client progress and changes in care needs
- Provide guidance and training to Personal Care Aides based on client care plans
- Observe and verify Personal Care Aide skills and competencies as required
- Assist with personal care needs when necessary
- Travel to clients' homes throughout the designated service area
- Valid nursing license in the Commonwealth of Virginia
- Home care or similar healthcare experience preferred
- Strong assessment, communication, and documentation skills
- Compassionate and professional approach to client care
- Ability to work independently and manage a flexible schedule
- Comfortable utilizing electronic documentation systems such as Axis Care
- Valid driver's license and reliable transportation
- Ability and willingness to travel to client homes throughout the service area
- Comfortable visiting homes that may have pets or smoking
- Part‑time and flexible scheduling
- Mileage reimbursement for work‑related travel
