The Norfolk Community Services Board (NCSB) provides treatment, support, and education to the community with a focus on mental illness, substance use addiction, and intellectual/developmental disabilities. Norfolk CSB provides services to persons in acute crisis as well as integrated, person-centered, and recovery-oriented programming for persons needing assistance and support stabilizing in the community.
About the Role:Family Services Worker I
Today, the NCSB provides treatment and rehabilitation services to over 6,000 Norfolk citizens yearly. Norfolk CSB seeks team members interested in providing skilled services in a public service environment. We are seeking a Family Services Worker I to provide initial outreach to homeless individuals and families in response to referrals from the community. The role includes assisting consumers in accessing needed services to include housing, medical, social, educational, vocational, and other supports necessary to meeting basic needs. The position requires completing documentation in accordance with agency guidelines.
Department Hiring Salary Range: $54,190- $62,185
Essential Functions- Conducting street outreach to homeless individuals in the city of Norfolk, attempting to develop relationships with people experiencing homelessness, and completing a face-to-face assessment to determine functional limitations, eligibility for service, progress, and need areas across street contact, site visits, office visits, or other community settings.
- Determining eligibility for enrollment and enrolling as appropriate, developing a Plan of Action based on assessment information in collaboration with the consumer and, if appropriate, family or significant others, and reviewing and updating plans as needed.
- Linking to shelter, services, resources, and supports specified in the Plan, including housing, entitlements, medical care, mental health services, substance use services, food, skill training, etc.
- Assisting the individual directly, which may include site visits for clients who are sheltered within the program and transportation to obtain needed resources, advocating for the consumer to procure needed resources or services while empowering the individual to be as independent as possible.
- Coordinating services and service planning with other providers within NCSB and with other agencies or private providers, such as hospitals, counselors, vocational programs, doctors, etc., obtaining clinical documentation from other providers to ensure continuity of care.
- Contacting family or significant others to promote implementation of ISP and community stability.
- Monitoring progress, service delivery, physical and mental stability, and consumer satisfaction by providing monthly service to all individuals on the assigned caseload and increasing frequency of service whenever clinically indicated.
- Providing practical guidance and education to guide the individual in problem solving strategies and develop a supportive therapeutic relationship; encouraging compliance with treatment, discussing relapse prevention strategies, stress management, etc.; and working to resolve critical situations or linking to emergency services as needed.
- Maintaining client records that comply with agency guidelines and state regulations, completing an accurate record of direct and consumer-related service provided daily.
- Complying with regulations for HIPAA, mandated reporting of abuse and neglect, consumer confidentiality, and other consumer rights.
- Attending agency staff meetings, case review meetings, supervisory meetings, and training as required.
- Performing other related duties as assigned.
- Minimum of a Bachelor's degree in a Human Services field or a Bachelor's degree in any field with a minimum of two years of appropriate and related experience in a Human Services area (Section 22
VAC
40-670-20 of the Administrative Code of Virginia). - Possession of a BSW or MSW degree and a Commonwealth of Virginia Social Worker license is desirable.
