Position: Medical Social Worker (LSW/LCSW) - BroMenn - Social Services at Carle Health Normal, IL

Overview

Provides social work services to patients and families within the hospital setting to meet identified psychosocial needs, enhance problem‑solving and coping capacities, and facilitate timely discharge by arranging post‑discharge services. The social worker conducts psychosocial assessments, offers supportive counseling, crisis intervention, connects with community resources, and collaborates with the healthcare team to ensure clear communication and comprehensive care planning.

Responsibilities
  • Work collaboratively with the RN Case Manager to design discharge plans based on assessment of patient and family needs, preferences, and available resources.
  • Identify insurance coverage and determine need for financial assistance for patients admitted with “no insurance” or “self‑pay.”
  • Respond to referrals from healthcare team members to locate facilities and services for case‑specific needs during and after hospitalization.
  • Evaluate patients for abuse or neglect and make appropriate referrals.
  • Involve patients and families in case planning decisions.
  • Advocate for patients in obtaining financial resources and government entitlements.
  • Provide information and counseling about advance directives, powers of attorney, and conservatorship.
  • Support patients and families in situations involving death, adoption, or fetal demise.
  • Offer counseling and coaching on coping with loss, disability, separation, financial impact, and traumatic life changes.
  • Inform director/manager of problematic cases, especially those involving legal or risk‑management issues.
  • Assess uninsured/underinsured patients for Illinois Medicaid eligibility or Carle Financial Assistance Program eligibility and assist with application(s).
  • Conduct psychosocial assessments to identify expressed, implied, or anticipated needs.
  • Complete mandated reports of suspected abuse and/or neglect of minors, disabled adults, and seniors.
  • Collaborate with investigative bodies to develop safe plans.
  • Help patients obtain prescription medications and related assistance programs (e.g., coupons, copay cards, pharmaceutical assistance, Social Security Extra Help, 340B, One‑Time Rx Assistance).
  • Arrange transportation for patients to medical appointments or home from the Emergency Department/Hospital when possible.
  • Educate and connect patients with community resources such as non‑skilled home help, meals, housing assistance, charity programs, and support groups.
  • Provide empathetic listening and emotional support in person or by phone.
Qualifications
  • Licensed Social Worker (LSW) or Licensed Clinical Social Worker (LCSW) from the Illinois Department of Financial and Professional Regulation.
  • Bachelor’s or Master’s Degree in Social Work or related field.
  • Experience working in a hospital or healthcare setting (Carle Health experience preferred).
Compensation

The hourly pay range for this position is $28.82 to $49.57 per hour, depending on experience, qualifications, and other factors.
