Overview
Sign-on Bonus & Relocation Bonus available. Evaluate and treat patients of all ages, modifying plan of care as appropriate. Serve as a role model in the delivery of professional services and as a clinical resource for staff and students.
Qualifications- Basic Life Support (BLS) certification within 30 days (American Heart Association).
- Licensed Physical Therapist (PT) within 90 days (Illinois Department of Financial and Professional Regulation).
- Bachelor's Degree in Physical Therapy (required).
- Appropriate work experience (details provided upon application).
- Perform patient transfers with assistance as needed. Demonstrate safe body mechanics to reduce risk of injury.
- Treat patients and staff with dignity and respect; remain empathetic and fully engaged.
- Develop and adjust individualized plans of care integrating goals, treatment, outcomes, and discharge planning.
- Educate patients, families, caregivers, students, and health care providers using effective teaching methods.
- Evaluate clinical findings to determine diagnoses and outcomes. Perform examinations using reliable, valid methods based on chief complaints.
- Document care in accordance with Clinical Process Policy (CTSCP
67). Produce documentation and charge in therapy EMR. - Screen patients to determine effectiveness and need for further therapy services.
- Use support personnel per legal and ethical guidelines; monitor their care and provide feedback.
- Evaluate impairments related to orthopedic and neurologic conditions. Establish plan of care based on functional deficits and patient goals.
- Complete objective testing for range of motion, strength, and functional tasks. Apply taping and modalities as appropriate.
- Communicate with referring providers on patient progress and treatment recommendations.
Hourly rate range: $36.74 - $63.19. Compensation depends on experience, qualifications, location, and other factors. Comprehensive benefits package available (visit careers.carlehealth.org/benefits for details).
