The Home Care Occupational Therapist evaluates and develops treatment plans for patients of all ages to resume or adjust the activities of daily living. Collaborates with the health care team and modifies the plan of care as appropriate. Serves as a role model in the delivery of professional services and as a clinical resource for staff and students. Hours may vary depending on census and program need.

Certifications:

Basic Life Support (BLS) within 30 days, American Heart Association (AHA);
Driver's License, Illinois Secretary of State (ILSOS);
Licensed Occupational Therapist (OT), Illinois Department of Financial and Professional Regulation (IDFPR);
Proof of Auto Insurance – varies.

Bachelor’s Degree in Occupational Therapy (required).

• Review the plan of care and orders with the primary physician, ensure written read‑back, complete immediately, and transmit within the time frame established by policy.
• Educate patients and family members in a manner that overcomes barriers, matches their learning capabilities, and meets fundamental needs.
• Document through comprehensive detail the patient visit so that following staff can determine exactly what and how care was rendered, with a clear plan for the next contact.
• Document within guidelines and time frames set by regulatory requirements and/or department policy.
• Update goals promptly, ensuring they are closed as met or changed as patient condition progresses.
• Collaboratively communicate and initiate case conferencing as necessary, documenting interactions with other disciplines, MDs, care coordinators, insurers, etc.
• Document care plans and patient/family assessments, demonstrating evidence of therapy knowledge, assessment skills, and critical thinking.
• Anticipate and order an appropriate quantity of supplies with sufficient lead time to prevent shortages between visits.
• Educate and document discharge planning prior to discharge, including communications with physicians, insurance sources, outpatient visits, and community resources, and arrange them if applicable.
• Provide discharge audits and other assigned tasks within the assigned time frame.
• Evaluate the patient’s level of function within regulatory requirements by applying diagnostic and prognostic procedures directed by the physician or patient.
• Observe, record, and report to the physician or podiatrist the patient’s reaction to treatments and any changes in condition.
• Instruct other health team personnel—including, when appropriate, Occupational Therapy Assistants, Home Health Aides, and family members—on the occupational therapy phase in which they may work, prepare written instructions, and supervise them in the patient’s home.
• Have an understanding of all home‑care rules and regulations and comprehend their influence on case‑mix index.
• Complete and document OTA supervisory visits as required (at least every six visits).
• Complete HCA supervisory visits every fourteen days if only therapy is provided in the home.
• Complete a comprehensive OASIS assessment of patients and assess home‑bound status.
• Complete a comprehensive occupational therapy evaluation in the patient’s home and develop a plan of care.
• Communicate with patients to schedule appointment times and manage the daily schedule.
• Assess home safety and recommend equipment to improve patient safety.
• Educate family/caregivers on recommended home exercise programs.

The compensation range for this position is $34.66 per hour to $59.62 per hour. This represents a good‑faith minimum and maximum range for the role at the time of posting. The actual compensation offered to a candidate will depend on a variety of factors including, but not limited to, the candidate’s experience, qualifications, location, training, licenses, shifts worked and compensation model.
