Overview
The purpose of this position is to help achieve the vision of Carle Home/Hospice Services by performing the duties of this position in a cost effective, results oriented and customer focused manner. This position is a critical component of the team structure within the Service Office and as such meeting individual performance and contribution to team performance goal is required. Provide database management and maintenance to daily data entry activities and maintain the data in the Epic computer systems.
Assist clinical field staff ordering medical supplies and DME. Prepare for and attend Team Meetings. Ensure all paperwork is completed. Appropriately triage incoming calls.

Maintains patient and staff schedules. Verifies daily schedules, updates schedules and productivity report daily. Check all scripts for input errors and corrects. Informs clinical manager of any variation in schedules. Advises home health aides of patient assignments. Validates schedule information for home health aide associates. Corrects schedule information and informs clinical manager as visit changes are needed. Contacts patient with any schedule variations as needed.
Ensures coverage of home health aides for all patient visits. Responsible for coordination of patient information. Insures compliance with HIPAA guidelines. Enters patient demographic changes. Responsible for maintenance of patient’s medical records with established policies and procedures. Fax/mail physician orders and track in electronic record to ensure timeliness of returned signed paperwork. Run active patient report daily. Fax discharge summaries to attending physicians.
Prepare agenda for team meeting. Pull together all information that is needed for that week’s team meeting. Attend the team meeting and ensure that all processes are followed and paperwork is completed to meet regulatory needs. Assists in maintaining inventory of medical supplies. Responsible for ordering medical supplies and following up with delivery issues. Run recertification list, copy and distribute to physician and staff.
Answering incoming calls from patients and families. Ensuring information is passed on to the appropriate clinician to assist with requests and issues. Communicating with staff to ensure all patient scheduling needs are fulfilled. Assisting with other duties as assigned.
The compensation range for this position is $18 per hour - $29.13 per hour. This represents a good faith minimum and maximum range for the role at the time of posting by Carle Health. The actual compensation offered a candidate will be dependent on a variety of factors including, but not limited to, the candidate’s experience, qualifications, location, training, licenses, shifts worked and compensation model.
