Social Worker; LSW​/LCSW Hospice

The role involves providing psychosocial assessments and interventions to patients and families in a hospice setting, addressing emotional, social, and environmental needs related to diagnosis and care. Responsibilities include developing and implementing care plans, supporting family coping, facilitating communication about life care decisions, and coordinating with interdisciplinary teams.
