One to One Health is looking for a talented and compassionate Provider to join our team in Normal, IL. (Relocation assistance available to eligible applicants). This position provides direct and indirect patient care and plays an integral role in the clinic’s success. Responsibilities include evaluating and treating acute and chronic health problems using evidence‑based protocols, and creating and implementing appropriate treatment plans under the supervising physician.
ESSENTIALFUNCTIONS
- Perform pre‑employment and annual wellness physicals
- Manage acute and chronic health problems using evidence‑based protocols and create/implement treatment plans under supervising physician’s protocols
- Provide health screenings and other disease prevention programs
- Evaluate and treat patients for injury and illness
- Report and review cases with the medical director and client managers
- Oversee respirator and auditory testing for employees by clinic staff
- Provide age‑appropriate assessments and interventions to meet the needs of teammates
- Uphold high ethical standards and practice within the scope of practice outlined in the written protocol
- Provide patient education, health coaching, and patient health advocacy
- Order and analyze diagnostic tests
- Monitor effectiveness of interventions
- Collaborate with multidisciplinary team members by making appropriate referrals
- Enter patient data into the EMR system accurately and promptly
- Assist in all areas of the clinic, if needed
- Perform related duties as required
- Board Certified Nurse Practitioner or Certified Physician Assistant with an active license, 1 year experience in family medicine, internal medicine or urgent care setting required
- DEA license preferred
- BLS Certified
- Comfort and confidence engaging with senior stakeholders, including client and leadership and interdisciplinary team members
- A passion for helping people and a desire to realize patient‑first healthcare
- A self‑starter, energized by dynamic operations and willingness to tackle a wide range of challenges
- The desire and ability to lead with compassion
- Ability to read, write, and communicate in English
- Excellent interpersonal and customer service skills
- Ability to effectively communicate orally and in writing
- Ability to follow written step‑by‑step instructions
- Ability to concentrate, think, and learn
- Physical ability to hear, see, sit, stoop, kneel, crouch, reach, lift, push, and pull
