Overview

The Licensed Practical Nurse (LPN) assumes responsibilities for the direct nursing care of assigned patients under the supervision of a registered nurse (RN) in patient care departments. Provides nursing services to patients and families in accordance with the LPN scope of practice as defined by the Illinois Board of Nursing. The LPN will be paired with an RN or UNL/Charge Nurse to assist with the patient load.

LPN tasks may include oral meds, IV piggyback meds, trach care, dressing changes, wound care, hourly rounding, reinforcing patient education, and completion of discharges. The LPNs will document assessments and care plans after the RN has completed the initial assessment and initiated the care plan.

Qualifications

Certifications:

Licensed Practical Nurse (LPN) - Illinois Department of Financial and Professional Regulation (IDFPR);
Basic Life Support (BLS) within 30 days - American Heart Association (AHA).

Education:

College Diploma:
Licensed Practical Nurse (Required). Certificate of Completion:
Licensed Practical Nurse.

Work Experience:

Responsibilities
  • Provide direct nursing care to patients under the supervision of a registered nurse.
  • Nursing care is guided by the provider's orders and the nursing plan of care within the LPN's scope of practice.
  • Report patient response to a registered nurse for evaluation, intervention, and modification of the plan of care.
  • Administer medications, including IV piggybacks and fluids but excluding IV push medications.
  • Document assessments on assigned patients, given that an RN has charted the initial assessment.
  • Chart on care plans, which the RN has opened.
  • Communicate with physicians/APPs to receive orders and coordinate care for the patients.
  • Complete dressing changes and wound care.
  • Complete trach care.
  • Insert NG tubes and maintain feedings through NG tube.
  • Care for and change ostomies.
  • Reinforce education and discharge instructions.
  • Fulfill all requirements of departmental competencies.
  • Takes responsibility for patient and employee safety by reporting and correcting safety hazards/incidents.
  • Perform all responsibilities and duties required by the unit as specific to the scope of service.
  • Work with a nurse preceptor to perform all responsibilities required by the unit and patient population.
  • Accurately and timely collect and document data of nursing care rendered to assure integration of information for use by the healthcare team.
  • Report adverse events to improve quality and safety for patients and staff.
  • Receive constructive feedback based on performance and behavior.
  • Regularly confer with colleagues to ask questions and expand knowledge base.
  • Complete all mandatory training and competency modules.

Compensation and Benefits

The compensation range for this position is $25.73 per hour - $41.87 per hour. This represents a good faith minimum and maximum range for the role at the time of posting by Carle Health. The actual compensation offered to a candidate will be dependent on a variety of factors including, but not limited to, the candidate’s experience, qualifications, location, training, licenses, shifts worked and compensation model.
