Health Physical Therapist (PT) Job Overview

We are seeking a compassionate and skilled Physical Therapist to provide quality in‑home nursing care for patients. In this role, you will coordinate patient care, perform skilled nursing procedures, and work closely with an interdisciplinary team to ensure the best possible outcomes.

Home Health Physical Therapist (PT)

Key Responsibilities
  • Evaluate patients and develop individualized treatment plans
  • Provide physical therapy services in accordance with physician orders
  • Monitor patient progress and modify care plans as needed
  • Complete accurate and timely documentation
  • Educate patients and caregivers on exercises and equipment use
  • Coordinate care with physicians and healthcare team
  • Supervise PTAs and aides as needed
  • Follow infection control and safety protocols
Home Health Physical Therapist (PT)

Minimum Requirements
  • Degree in Physical Therapy from an accredited program
  • Current state PT license or compact license
  • At least 1 year of relevant experience preferred
  • Current CPR certification
  • Strong communication skills
Work Environment
  • Care provided in patient homes under varying conditions.
  • Exposure to infectious diseases, blood, and bodily fluids possible.
  • Requires local travel and ability to work a flexible schedule.
  • May involve prolonged standing, walking, and lifting up to 50 lbs.

Pay: $70,000.00 - $85,000.00 per year
