Physical Therapy - PT SNF
Physical Therapist (PT) permanent placement opportunity in a Skilled Nursing Facility (SNF) and VA setting with a Monday–Friday schedule from 8:00 AM to 4:30 PM. This role offers approximately 40 hours per week with no weekend rotation, providing a stable work-life balance in a rehabilitative care environment. The PT will evaluate and treat patients with a focus on improving mobility, strength, balance, and overall functional independence.
This position is ideal for clinicians seeking long-term placement in post-acute and veteran-focused care settings.
Requirements:
- Minimum 2 years of Physical Therapist experience
- Active State PT License required
- BLS certification required
- Experience in SNF, rehab, or VA setting preferred
- Strong knowledge of therapeutic exercises, mobility training, and functional rehabilitation
- Ability to work independently and manage a consistent caseload
- Excellent communication and interdisciplinary collaboration skills
- Ability to provide compassionate, patient-centered care
The Physical Therapist will evaluate patients, create individualized treatment plans, and implement therapy interventions aimed at restoring functional independence and improving quality of life. Responsibilities include therapeutic exercises, gait training, transfer training, balance activities, and patient mobility assessments. The therapist will monitor patient progress, adjust treatment plans as needed, and document all care in compliance with facility and regulatory standards. Additional duties include patient and family education, collaboration with interdisciplinary healthcare teams, and supporting discharge planning to ensure continuity of care and optimal outcomes.
