Senior Case Manager

The College of Nursing is seeking a dedicated and experienced Senior Case Manager to join our Case Management Department supporting home and community-based waiver programs. This position plays a critical role in coordinating services for aging and disabled populations while helping individuals maintain independence and quality of life within their homes and communities.

• Manage a full-time caseload and serve as an experienced resource within the department, providing guidance, support, and leadership to team members while ensuring high-quality case management services are delivered in accordance with organizational standards and program requirements.
• Work independently under limited supervision and be responsible for all aspects of the case management process, including admissions, assessments and reassessments, service planning, monitoring, coordination of care, and discharge planning.
• Develop and maintain meaningful relationships with clients, caregivers, providers, and interdisciplinary teams.

• Strong critical thinking, communication, and organizational skills.
• One year paid professional experience with the aging adult or disabled population.
