Frontier Hospice is seeking a PRN Hospice RN Case Manager (RNCM) to join our dedicated nursing team providing exceptional end-of-life care throughout the community.

Oklahoma City and surrounding areas.
In this rewarding role, you’ll deliver hands-on hospice nursing care, coordinate services, and guide patients and families through every step of their hospice journey. - Deliver hands-on hospice nursing care, coordinate services, and guide patients and families through every step of their hospice journey.
- Collaborate with our interdisciplinary care team to ensure every patient receives care grounded in compassion, clinical excellence, and connection.
- Respond to clinical referrals with care and expertise, addressing inquiries and requests with a compassionate touch.
- Partner with the Clinical Director to assess eligibility and suitability of clients for our specialized hospice services.
- As an RN Case Manager, coordinate care to maintain patient comfort and quality of life.
- Deliver nursing care rooted in proven principles and techniques to ease pain and bring solace.
- Educate and advocate for patients and families to create personalized care plans aligned with their goals.
- Participate in call duties as part of the team’s coverage when needed.
- Hold a current unencumbered license as a Registered Nurse.
- Minimum one (1) year of RN experience in a medical-surgical/acute care setting; hospice experience a plus.
- Must be computer proficient, including EMR experience.
- Possess and maintain current CPR certification if required by state.
- Current driver s license, vehicle insurance, and dependable transportation.
- Current, unencumbered RN license.
- Minimum one year of RN experience in medical-surgical/acute care; hospice experience preferred.

Compensation: $36.00 - $40.00 per hour (Average Pay Range). The pay range listed represents a general posting guideline. Final compensation will be determined based on experience and responsibilities, and discussed at the conclusion of the interview process.
