HOSPICE LICENSED PRACTICAL NURSE (LPN)

A Path of Care Hospice in Tulsa, OK is seeking a full‑time Hospice Licensed Practical Nurse (LPN) to provide compassionate, skilled care to patients in their homes. This role is ideal for an LPN who wants to grow professionally while working with a dedicated, mission‑driven healthcare team.

This position offers competitive pay based on experience and a $3,000 loyalty bonus, along with medical, dental, vision, life insurance, a 401(k), PTO, six paid holidays, optional Allstate insurance, and flexible scheduling.

What You'll Do as a Hospice LPN

As a Hospice LPN, you provide direct patient care under the supervision of the RN Case Manager and play a vital role in supporting patients and families. Your responsibilities include:

  • Delivering care according to the individualized plan of care
  • Assessing patient needs and reporting changes in condition
  • Identifying and reporting any signs of abuse
  • Making recommendations for updates to the plan of care
  • Counseling patients and families on advance directives such as living wills and healthcare surrogate forms
  • Maintaining accurate, up‑to‑date patient records
  • Building positive relationships with vendors and community partners
  • Participating in Total Quality Improvement initiatives and training programs
  • Assisting with new nurse orientation and discharge planning
  • Participating in the on‑call rotation as needed

Your compassion and clinical skill help patients and families navigate end‑of‑life care with dignity and support.

Qualifications
  • Active Oklahoma LPN license
  • At least 1 year of clinical experience
  • Ability to care for patients and families facing terminal illness
  • Valid Oklahoma driver's license, reliable transportation, and vehicle insurance
  • Acute care experience preferred but not required
  • Strong interpersonal and communication skills
  • Ability to work effectively as part of a team
Work Schedule
  • Full‑time
  • Monday‑Friday, 8:00 AM - 5:00 PM
