Shift: 12‑hour shifts – 6:30 a.m. – 7:00 p.m. or 6:30 p.m. – 7:00 a.m.
Salary: $45,760.00 per year ($22.00 hourly)
Shift differential: $0.87 per hour for evening and night shifts only.
Biweekly benefit allowance (2026): health, dental, vision, disability, life insurance, supplemental life insurance, dependent life insurance, flexible spending, and daycare account. Employee only – $377.15, Employee & Spouse – $687.98, Employee, Spouse & Child – $821.34, Employee, Spouse & Children – $914.28, Employee & Child – $510.51, Employee & Children – $603.45.
Retirement:
Pathfinder 401(k) with 3 plan choices – Standard (4.5 % match, up to 6 %), High (7 % match, up to 7 %), Customized (any amount over 4.5 % or 7 %; company match 6 % for 7 %). Vesting: 20 % per year, reaching 100 % after 5 years.
Longevity bonus: starts after 2 years with payments increasing every 2 years: $250.00 (2–4 years), $426.00 (4–6 years), $626.00 (6–8 years), $850.00 (8–10 years), $1,062.00 (10–12 years), $1,250.00 (12–14 years), $1,500.00 (14–16 years), $1,688.00 (16–18 years), $1,900.00 (18–20 years), $2,000.00 (20 years+). For each additional 2 years after 20 years, $200 is added.
Paid Time Off- Annual leave – 1‑5 years: 15 days a year possible (4.62 h biweekly), 5‑10 years: 18 days a year possible (5.54 h biweekly), 10‑20 years: 20 days a year possible (6.15 h biweekly), 20+ years: 25 days a year possible (7.69 h biweekly).
- Sick leave – 15 days a year possible (4.62 h biweekly).
- Maximum accruals and payouts:
- 0–5 years: 360 h accrual, 240 h payout
- 5–10 years: 784 h accrual, 640 h payout
- 10–20 years: 800 h accrual, 640 h payout
- 20+ years: 840 h accrual, 640 h payout
Holiday leave – 11 holidays accrued yearly, usable within 6 months of accrual.
Admin weather leave – essential personnel accrue up to 8 h per day or 32 h total for work during inclement weather, usable within 6 months of accrual.
Military leave with pay – maximum 30 days (240 h) per federal fiscal year, starting Oct 1–Sept 30. Must provide military orders.
Job Description Basic PurposeProvide direct and indirect non‑professional nursing care or outreach services in a clinic, hospital, institution or home setting under nursing or medical supervision. Tasks include vital sign measurement, lab specimen collection, charting, patient escort, assist with activities of daily living, and supply order and receipt.
Typical Functions- Perform direct nursing tasks: vital signs, activities of daily living, hygiene needs, and lab specimens.
- Chart observations and activities; report relevant changes in patient condition.
- Execute delegated or other specialized functions as educationally prepared.
- Escort patients to and from destinations.
- Maintain a clean, safe environment.
- Order, receive, store supplies; perform basic clerical functions.
- Participate in follow‑up activities, track clients, schedule appointments and other related tasks.
- Provide case finding and recruit at‑risk persons for comprehensive health services.
Full range of patient care duties or services under nursing or medical direction, including vital signs, lab specimen collection, activities of daily living, observation and charting, reporting information, and outreach services to community clients.
Education and ExperienceCurrent Long‑Term Care (CNA) certification. One year of patient‑care experience.
Knowledge, Skills, Abilities and CompetenciesKnowledge of English language, public health services, record keeping, interviewing techniques, basic nursing practices, sanitation and personal hygiene, and medical terminology. Ability to record observations, follow oral and written directions, establish effective relationships, and apply basic nursing techniques.
