Haven Home Health & Hospice is seeking a compassionate and organized Clinical Coordinator (LPN) to join our team. The Clinical Coordinator serves as a vital link between patients, caregivers, physicians, and clinical staff to ensure quality patient care and effective communication.

Process physician orders and patient care updates

Assist with patient admissions and ongoing care coordination

Monitor and communicate changes in patient condition

Maintain accurate patient records and documentation

Support clinicians with scheduling, orders, and care planning

Participate in interdisciplinary team communication

Assist with orientation and training of new staff

Participate in on-call, weekend, and holiday rotations as assigned

Qualifications

Current Oklahoma LPN license required

Home Health experience preferred

Previous clinical coordination experience preferred

Strong communication and organizational skills

Ability to multitask in a fast-paced environment

Proficient computer and documentation skills

Valid driver's license, reliable transportation, and current auto insurance

Schedule

Full-Time Monday-Friday On-call rotation required
