Family Home Health Network is seeking a dedicated Home Health Nurse in Illinois with a strong commitment to patient care. You will perform a comprehensive initial assessment of patients, families and home environments to determine care needs and develop plans of care.
You will monitor nursing needs, re-evaluate regularly and revise plans as patient status changes. A valid IL driver’s license, CPR and HCHB EMR experience are advantageous.
