• Assess Oregonians applying for or currently receiving long‑term care services to develop, coordinate, and arrange appropriate services.
• Provide choice counseling and develop individualized case management plans for in‑home, community‑based, and nursing home care.
• Coordinate provision of services, regularly review and update long‑term care service eligibility, monitor social service plans, and identify and mitigate safety risks.
• Contact Oregonians or collateral contacts monthly via in‑person, telephone, or electronic communication to review service plans and safety needs.
• Ensure service delivery is culturally competent, trauma‑informed, and available in multiple languages and formats; facilitate bilingual services and accessibility for all.
• Work collaboratively with the eligibility team and use the ONE System to review and update Long Term Care Services and provide information to Oregonians.
• Interview Oregonians to determine initial and ongoing eligibility for healthcare, social services, and protective service needs, including collateral interviews.
• Develop detailed, person‑centered case plans and conduct assessments using the Case Assessment and Planning System (CA/PS) on a state‑issued laptop.
• Process applications, inquiries, and data according to policy time frames, ensuring documentation and forms are completed, sent, and eligibility decisions communicated.
• Collaborate with Adult Protective Services, AFH Licensors, and other agencies to ensure the safety and protection of Oregonians and comply with mandatory reporting guidelines.

• At least three years of human services related experience, OR a bachelor’s degree in Behavioral Science, Social Science, or a closely related field.
• Alternatively, an associate degree in Behavioral Science, Social Science, or a closely related field plus two years of human services experience.
• Or an equivalent combination of experience, education, and training that meets the role’s requirements.

• Experience coordinating services and support for individuals, assessing needs, developing service plans, monitoring progress, and adjusting services.
• Experience interpreting and applying laws, regulations, policies, or procedures to make consistent, accurate, and well‑supported decisions.
• Strong documentation skills; ability to prepare clear, accurate, and timely records that support decisions and comply with organizational or legal requirements.
• Experience conducting interviews or information‑gathering conversations to assess needs, evaluate available information, and determine appropriate next steps.

Your work schedule will be 8:00a.m. – 5:00p.m., Monday – Friday Pacific Standard Time (PST) with occasional overtime and flexible scheduling as a condition of employment. The position is assigned to the Store Front Office, primarily office and community work, with possible travel for business needs or training. A valid Oregon driver’s license and acceptable driving record are required for use of state vehicles.

The role may involve frequent phone use, electronic data processing, and interaction with individuals who have experienced trauma.

Selected finalists will undergo a criminal history and background check; adverse findings may lead to disqualification. The Oregon Department of Human Services does not offer visa sponsorship. Within three days of hire, candidates must complete the U.S. Department of Homeland Security I‑9 form and pass E‑Verify to confirm authorization to work in the United States.
