About This Role
The Wound Care Coordinator provides evidence-based practice to deliver excellence in prevention, health maintenance, therapeutic intervention, rehabilitative, and palliative nursing care for patients with gastrointestinal, genitourinary, and integumentary disorders. This role is ideal for experienced nurses who are passionate about improving patient care and outcomes.
Highlights- Location: North Bergen, NJ
- Facility: Hackensack Meridian Health
- Schedule: Full-time, Day shift
- Serve as a consultant for complex clinical cases to improve patient care and outcomes.
- Use judgment and critical thinking to assess, diagnose, and develop individualized care plans.
- Implement and evaluate care for patients with wounds, ostomy, or continence issues.
- Lead and participate in inter-professional teams to ensure patient-centered care.
- Develop, evaluate, and revise processes and protocols based on evidence‑based practice.
- Collaborate with Nursing Education to educate staff on wound care.
- Monitor patients with pressure injuries in collaboration with relevant departments.
- Evaluate new products and ensure inventory meets patient and nursing staff needs.
- Communicate effectively with patients and caregivers at their level.
- Adhere to HMH Organizational competencies and standards of behavior.
- Bachelor's of Nursing required
- Completion of a WOCN Society accredited WOC education program within 18 months of employment
- Minimum of 3 years nursing experience required
- Current NJ State Professional Registered Nurse License
- AHA Basic Health Care Life Support HCP Certification required
- Wound, Ostomy, and Continence Certification within 18 months of entering position
