Pathway Geriatrics seeks an experienced Geriatric Nurse Practitioner to provide palliative primary care for homebound older adults across Edmonds/Lynnwood/Bothell regions. The role is 100% field-based with weekly meetings at the Redmond office.
You will visit 1–5 homes weekly, manage medications between visits, and coordinate with families and caregivers to optimize quality of life while reducing unnecessary ER visits.
