Licensed Practical Nurse (LPN) (North Branch, MN)

Residential Services, Inc. (RSI) is seeking a full-time Licensed Practical Nurse (LPN) to support programs in East Central Minnesota. This role provides nursing services to individuals with a wide range of developmental, physical, and mental health needs. Travel between locations is required with mileage reimbursement provided.

Salary: $26.00-$27.00 per hour.
Shifts: Monday through Friday with flexible hours between 8:00 am‑5:00 pm. 40 hours per week. Driver’s license and reliable transportation required.

Position Summary

The Licensed Practical Nurse (LPN) supports the coordination and delivery of healthcare services for individuals across multiple programs. This role works closely with the Registered Nurse and interdisciplinary teams to ensure quality care, regulatory compliance, and effective staff training.

Key Responsibilities
  • Monitor health conditions and report changes to the RN
  • Coordinate care with physicians, pharmacy, therapy, and other providers
  • Administer, track, and oversee medication processes and documentation
  • Assist in developing and updating care plans and health assessments

    Train and support staff on medication administration, medical procedures, and infection control
  • Ensure accurate, timely documentation and compliance with regulations
  • Participate in interdisciplinary team meetings and provide health updates
  • Conduct audits, track appointments, and monitor follow‑up care
  • Support infection control practices and program quality assurance
  • Provide general nursing care to people receiving services
  • Participate in on‑call rotation and travel for trainings/meetings as needed
Qualifications
  • Valid, unrestricted driver’s license
  • Current Minnesota LPN license and completion of an accredited practical nursing program
  • At least 2 years of nursing experience preferred, especially working with individuals with varying abilities
  • Willingness to complete and pass a background check and MVR screening after offer
