Position: Licensed Practical Nurse LPN, Full Time

The Licensed Practical Nurse (LPN) is responsible for providing nursing care to residents and patients, including medications, treatment administration, documentation, and other therapeutic interventions under the direction of the Director of Nursing/Clinical Director. The position directs Nursing Assistants, Resident Assistants, Trained Medication Assistants, and other clinical staff on unit or shift.

Schedule
  • Full‑time
  • Shift: 2:30pm‑11:00pm or 10:30pm‑7:00am
  • Works various weekdays, every other weekend & rotating holidays.
Pay
  • Targeted range: $31.89 to $34.28 per hour.
  • Shift differential for evenings ($1.00 per hour) and overnights ($2.00 per hour).

Job Responsibilities
  • Provides nursing care within the scope of practice of an LPN.
  • Coordinates and directs resident and patient care, collaborating with other departments and providers from admission to discharge.
  • Assists the Registered Nurse with assessments, documentation, and data collection within scope of practice, acting on findings in a timely manner.
  • Administers medications and completes treatments as ordered; observes and monitors residents’ condition, reporting changes as appropriate; ensures physician orders are signed and implemented within specified times.
  • Maintains the safety of all residents, patients, and team members; responds to safety hazards, reports issues to the appropriate person, and ensures staff follows infection control policy.
  • Maintains a clean, sanitary, safe, and efficiently organized work area.
  • Performs other duties as assigned.
Minimum

Required Qualifications
  • Degree from an accredited nursing school for the Licensed Practical Nurse program.
  • Current LPN licensure in the state where the role is located.
  • Effective written and verbal communication skills.
  • Basic computer skills.
  • Ability to follow all safety rules, regulations, policies, and procedures, including the Vulnerable Adult and Abuse Policy, HIPAA, Resident’s Bill of Rights, Universal Precaution, AWAIR, OSHA, Right to Know, SDS, ADA, HUD Policy and Procedures, and Fair Housing laws.
Preferred Qualifications
  • Previous experience providing senior care or in a health‑care setting.
