Licensed Practical Nurse
Group Home Setting Medically Fragile Individuals
$27.36 per hour
This is a fantastic role to continue your nursing career path and work within a setting that offers variety and rewarding challenges. You will have high-impact work by providing nursing services, training, and supports to address the medical or psychiatric needs of individuals with a brain injury, intellectual and developmental disability, a substance use disorder, and/or has an emotional or behavioral disability.
- Administer medications, treatments, and tube feedings as prescribed
- Monitor residents’ health status and promptly report changes or concerns
- Provide care for individuals with complex medical needs, including:
- Urostomy care
- Colostomy care
- G-tube/J-tube care
- Catheter care and irrigation's
- Monitoring vital signs and weights
- Oxygen therapy, CPAP, and Bi-pap support
- Educate DSP staff and family members regarding resident care needs
- Maintain accurate nursing documentation and medication administration records
- Coordinate medical appointments and communicate with physicians and healthcare providers
- Ensure compliance with state regulations, infection control standards, and agency policies
- Respond appropriately and efficiently to medical emergencies
- Assist with lifting, transfers, and resident mobility support as needed
- Current LPN license in good standing
- Strong clinical and documentation skills
- Experience working with medically fragile individuals, developmental disabilities, or TBI populations preferred
- Knowledge of medication administration and nursing procedures
- Excellent therapeutic communication and interpersonal skills
- Ability to prioritize tasks efficiently and work ethically and professionally
- Ability to work collaboratively within a team-oriented environment
- Compassionate, patient-centered approach to care
