Group Home Setting Medically Fragile Individuals
$27.36 per hour
This is a fantastic role to continue your nursing career path and work within a setting that offers variety and rewarding challenges. You will have high-impact work by providing nursing services, training, and supports to address the medical or psychiatric needs of individuals with a brain injury, intellectual and developmental disability, a substance use disorder, and/or has an emotional or behavioral disability.
Administer medications, treatments, and tube feedings as prescribed
Monitor residents' health status and promptly report changes or concerns
Provide care for individuals with complex medical needs, including:
Urostomy care
Colostomy care
G-tube/J-tube care
Catheter care and irrigation's
Monitoring vital signs and weights
Oxygen therapy, CPAP, and Bi-pap support
Educate DSP staff and family members regarding resident care needs
Maintain accurate nursing documentation and medication administration records
Coordinate medical appointments and communicate with physicians and healthcare providers
Ensure compliance with state regulations, infection control standards, and agency policies
Respond appropriately and efficiently to medical emergencies
Assist with lifting, transfers, and resident mobility support as needed
Qualifications
Current LPN license in good standing
Strong clinical and documentation skills
Experience working with medically fragile individuals, developmental disabilities, or TBI populations preferred
Knowledge of medication administration and nursing procedures
Excellent therapeutic communication and interpersonal skills
Ability to prioritize tasks efficiently and work ethically and professionally
Ability to work collaboratively within a team-oriented environment
Compassionate, patient-centered approach to care
