Role Overview

The RN Case Manager provides direct care in addition to overseeing and directing home health services for clients within the community. The RN collaborates with clients, families, and health care teams to develop and implement goal-centered care plans, consider quality measures and financial aspects of treatment, deliver skilled nursing care, and provide clinical oversight to professional and unlicensed personnel. The position reports to the Clinical Supervisor.

• This is a full‑time position, Monday through Friday with an on‑call rotation. RN Case Manager manages schedule during office hours.
• The role requires traveling to client homes to provide 1:1 client care, typically 1‑3 hours per client.

• Perform initial and periodic assessments to determine care needs, implement interventions, and evaluate outcomes.
• Collaborate with physicians, non‑physician practitioners and the home health team to update client home‑health plans at least every 60 days or more often as needed.
• Serve as client advocate, ensuring safety needs are addressed, assessing the home environment, developing abuse‑prevention plans, and reporting suspected abuse, neglect or financial exploitation.
• Meet documentation standards for accuracy, timeliness and compliance with payer/billing requirements.
• Provide education to clients, families, caregivers, licensed clinicians and unlicensed personnel.
• Deliver direct care to clients of varying ages, acuity levels and diagnoses per company policies, procedures and the Minnesota Nurse Practice Act.
• Coordinate client care with family, caregivers, community resources and other healthcare members.
• Provide clinical oversight, delegation and leadership to licensed clinicians and unlicensed personnel.
• Ensure effective financial management of client home‑health services.
• Manage work schedule effectively and remain flexible during unexpected changes and on‑call duties.

Client care occurs in individual homes and may require transfers and lifting with adaptive equipment. Must be adaptable to various environments, drive or use public transportation in all types of weather. Heavy: 50‑100 lb occasionally; 25‑50 lb frequently; 10‑20 lb constantly.

• Registered Nurse licensed in Minnesota, without revocation, suspension or restrictions.
• Proof of negative TB test:
  Mantoux skin test, single TB blood test or chest X‑ray.
• Access to a dependable automobile for multiple company business stops daily.
• Valid driver’s license and proof of current car insurance for personal vehicle use.
• U.S. citizenship or valid alien work permit.
• Disclosure of any convictions or criminal history related to health services; such conviction disqualifies the candidate.
• Pass initial and ongoing background checks, including those of the Minnesota Department of Health and the Federal Office of the Inspector General’s exclusion list.

• At least two years of experience in a healthcare facility or home‑care field.
• Demonstrates critical thinking, follows standards and procedures, and makes appropriate clinical judgments for safe client care.
• Works independently with minimal supervision, adapts to change, manages multiple assignments and prioritizes duties.
• Builds and maintains professional relationships.
• Excellent written and oral communication, interpersonal and teaching skills.
• Basic computer skills; willing to learn new software; experience with electronic medical records and Microsoft Outlook preferred.
• Current CPR certification highly recommended.

The base salary range for this position in the U.S. is $70,000–$95,000 per year plus productivity, compensation and benefits. Salary determined by experience, skills, certifications, job level, supervisory status, and location.
