Ecumen Hospice North Branch is hiring a Registered Nurse providing services in Chisago County and East Central Minnesota. The team focuses on life‑honoring care for patients north of the Twin Cities metro area, including Isanti, Chisago and Pine counties. Schedule

Part Time – Weekends only 8am-4:30pm every Saturday & Sunday.

Pay

The targeted pay range for this job is $38.50 – $43.50 per hour. Pay is based on experience, qualifications, certifications, licenses and education.

Essential

Job Responsibilities

Receives and responds to phone calls during assigned shifts, makes visits to patients with changing conditions, and completes weekend admissions.

Completes an initial, comprehensive and ongoing assessment of patient and family to determine hospice needs, provides a complete physical assessment and history of current and previous illness.

Assesses and evaluates patient status by writing and initiating a plan of care, re‑evaluating patient and family/caregiver needs on a regular basis, and revising the plan of care as necessary.

Develops a care plan that establishes goals based on nursing diagnosis and incorporates palliative nursing actions, including the patient and family in the planning process.

Administers medications and treatments as prescribed by the physician in the plan of care. Records pain and/or symptom management changes/outcomes as appropriate.

Initiates the plan of care and makes necessary revisions as patient status and needs change. Communicates with the physician regarding patient needs and reports changes in the patient’s condition; seeks physician’s orders as required.

Provides health‑care instructions to the patient as appropriate per assessment and plan.

Driving Requirements

Local travel and driver’s license required.

Registered Nurse HSP Minimum

Required Qualifications

Graduate of an accredited Registered Nursing program
1 year of nursing experience in a post‑acute care setting
Must be licensed as a Registered Nurse in the state where working and must maintain licensure
Valid driver’s license, proof of insurance and satisfactory motor vehicle record
Ability to communicate effectively both verbally and written
Proficient with Microsoft Office Suite
Able to follow all safety rules, regulations, policies and procedures of the facility including but not limited to Vulnerable Adult and Abuse Policy, HIPAA, Home Care, Bill of Rights, OSHA, Universal Precaution, AWAIR, Right to Know, SDS, ADA, HUD policy and procedures, Fair Housing laws and regulations

Preferred Qualifications

Bachelor of Science degree in Nursing
One year nursing experience in Hospice

Registered Nurse (required)
