Admissions Registered Nurse (RN) – Home Health Start of Care Specialist

This role focuses on high-volume admissions for home health patients. You will streamline admissions, conduct assessments, manage care plans, and coordinate with the care team to ensure smooth transitions.

• Complete Start of Care (SOC) admissions efficiently and accurately.
• Perform clinical assessments and OASIS documentation with speed and precision.
• Initiate care plans and identify needed services and next steps.
• Communicate effectively with the care team to keep patient transitions moving smoothly.
• Manage your schedule and territory to optimize time, travel, and visit volume.

• Active Minnesota RN license in good standing.
• Experience with home health admissions and OASIS strongly preferred.
• Ability to work independently in a fast-paced, field-based role.
• Strong time management and organizational skills.
• Reliable transportation.
