Licensed Practical Nurse

Evergreen Woods in Branford, CT is seeking a compassionate Licensed Practical Nurse to provide high-quality care to residents in our Life Plan Community. In this full-time role, you'll administer medications and treatments, monitor resident conditions, support assessments and care plans, and ensure accurate documentation. You'll collaborate closely with RNs, physicians, and caregivers while building meaningful relationships with residents and families in a warm, resident-first environment that supports your growth and development.

Responsibilities

• Administer medications and treatments per physician orders and facility protocols
• Monitor and document resident conditions, vital signs, and responses to treatment
• Assist with assessments and implementation of individualized care plans
• Maintain accurate, timely documentation in electronic health records
• Collaborate with RNs, physicians, CNAs, and interdisciplinary team members
• Provide direct resident care, including ADL support as needed
• Educate residents and families on care, treatments, and wellness practices
• Support infection control, fall prevention, and safety initiatives
• Respond to changes in resident status and emergencies promptly
• Contribute to a warm, respectful, resident-first environment
