About the RoleSt. Luke is seeking a dedicated, detail-oriented Wound Care Nurse to manage and oversee wound care operations across our sister facilities:
St. Luke North Canton and St. Luke Portage Lakes. In this dual-facility role, you will champion skin integrity and wound healing for our Skilled Nursing Facility (SNF) and Assisted Living Facility (ALF) residents.
If you are a clinical professional passionate about specialized care, tracking healing progress, and educating staff to prevent skin breakdown, we want you on our team.

Specialized Care: Assess, stage, and treat pressure ulcers, surgical wounds, diabetic ulcers, and other skin integrity issues across both the North Canton and Portage Lakes campuses.
Treatment Coordination: Administer wound care treatments, dressings, and therapies according to physician orders and established clinical protocols.
Collaboration & Documentation: Work closely with physicians, directors of nursing, and floor staff to update care plans. Maintain meticulous electronic documentation of wound progression.
Prevention &
Education:
Educate nursing staff, residents, and family members on proper positioning, skin care techniques, and preventive measures.Rounds: Conduct regular wound rounds and participate in quality assurance reviews to ensure optimal resident outcomes.
Active, unencumbered RN or LPN license in the state of Ohio.
Previous experience in wound care management is highly preferred (WCC, WOCN, or CWS certification is a major plus, but not required).
Prior clinical experience in a Skilled Nursing (SNF) or Assisted Living (ALF) environment.
Ability to travel reliably between our North Canton and Portage Lakes facilities.
Strong communication, assessment, and time-management skills.
